I can't speculate on the exact cause for the multi-level failure any more than I already did in the essay. But at the very least we had a swiss cheese model systems failure. Was there collusion by those charged with protecting the nation? We don't have concrete evidence to say so,.

one of the conspiracy theories during Covid was that the tests themselves where giving people Covid and or making them generally sick
if this operation was creating or distributing Covid kits etc god know what we could of gotten
